Payment 

Cash, check, or credit card (Visa, Master Card, Discover, and American Express).

Payment is requested at the time of session.  I am an out of network provider.  As a Licensed Mental Health Counselor, a majority of insurance companies will reimburse for a portion of my services. The reimbursable amount varies based on your mental health benefits.  I am able to submit claims on your behalf and if covered, your insurance company will reimburse you directly. In cases where I cannot electronically submit claims on your behalf, you will be provided documentation you will be able to submit for reimbursement.  If you are signing as the agent for the patient, you will be responsible for the full amount the patient must pay.


Insurance 

Out of network benefits for counseling / psychotherapy. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?

important to know regarding confidentiality 

Your health insurance company and / or a third party payer may require that I provide them with information about your diagnosis, treatment plan, and your attendance at therapy sessions.  If you are using insurance and / or a third party payer, you acknowledge this and you agree to allow these disclosures.


Cancellation Notice 

If you are unable to keep your appointment, please provide at least 48 hours notice or you will be charged the full cost of the session.
